An obviously thickened basement membrane has me thinking of lupus erythematosus. You do not usually see this feature in Dermatomyositis which is a common differential diagnosis. Also lupus tends to have a greater dermal inflammatory response than dermatomyositis. Rarely lichen sclerosus can show a thickened basement membrane.
